Where Insulin Actually Comes From
Insulin is produced by specialized clusters of cells inside the pancreas called the Islets of Langerhans — named after the German medical student Paul Langerhans, who first described them in 1869. These tiny islands make up only about 1–2% of the pancreas by mass, yet they carry enormous responsibility.
Within those islets, it's specifically the beta cells that manufacture and release insulin. When you eat and your blood glucose rises, beta cells detect the spike and respond by secreting insulin into the bloodstream. Think of insulin as a key that unlocks your cells, letting glucose in so it can be used for energy.
A Dual-Purpose Organ
What makes the pancreas fascinating is that it actually has two completely separate jobs. Its exocrine function — which accounts for most of its tissue — produces digestive enzymes that flow into the small intestine to break down food. Its endocrine function, handled by those islets, releases hormones like insulin and glucagon directly into the blood.
Glucagon, produced by alpha cells in the same islets, does the opposite job to insulin: it raises blood sugar when levels drop too low. The two hormones work as a finely tuned balancing act, keeping glucose in a safe range around 70–100 mg/dL when fasting.
When It Goes Wrong: Diabetes
The stakes become clear when the system breaks down. In Type 1 diabetes, the immune system mistakenly destroys the beta cells entirely, leaving the body unable to produce insulin at all. Patients require daily insulin injections or a pump to survive.
In Type 2 diabetes, the beta cells still function but the body's cells become resistant to insulin's signal. Over time, the overworked beta cells can begin to fail. More than 500 million people worldwide live with diabetes — a figure that underlines just how vital this small cluster of pancreatic cells really is.
A Century of Life-Saving Science
Before 1921, a Type 1 diabetes diagnosis was essentially a death sentence. That year, Canadian scientists Frederick Banting and Charles Best successfully extracted insulin and used it to treat a dying 14-year-old boy named Leonard Thompson. It worked. Banting received the Nobel Prize in Physiology or Medicine in 1923.
Today, insulin is one of the most widely used medicines on the planet. Modern versions are synthesized using genetically engineered bacteria — a far cry from the early extracts taken from animal pancreases. But it all traces back to that unassuming organ sitting quietly behind your stomach, doing its essential work right now.
