Tucked behind your stomach, roughly the size of your hand, sits one of the most hardworking — and underappreciated — organs in the human body. The pancreas quietly runs two completely separate jobs at once, and one of them keeps you alive every single time you eat a meal.
Where Insulin Actually Comes From
Insulin is produced by specialised clusters of cells within the pancreas called the Islets of Langerhans. Named after the German pathologist Paul Langerhans, who first described them in 1869, these tiny cell clusters make up only about 1–2% of the pancreas's total mass. Within each islet, it's specifically the beta cells that manufacture and release insulin directly into the bloodstream.
When you eat carbohydrates and your blood glucose rises, beta cells detect the spike and release insulin in response. Insulin then acts like a key, unlocking cells throughout your body so they can absorb glucose and use it for energy. Without it, glucose simply builds up in the blood — with seriously dangerous consequences.
A Dual-Purpose Organ
What makes the pancreas genuinely fascinating is that it operates as two organs in one. Its exocrine function — which accounts for the vast majority of its tissue — produces digestive enzymes that flow into the small intestine to break down fats, proteins, and carbohydrates. Its endocrine function, handled by those Islets of Langerhans, secretes hormones like insulin and glucagon directly into the blood.
Glucagon, produced by the alpha cells in the same islets, does the opposite job to insulin — it raises blood sugar when levels drop too low. The two hormones work in constant opposition, keeping your glucose levels in a remarkably tight range around 4–6 mmol/L.
When the System Breaks Down
In Type 1 diabetes, the immune system mistakenly destroys the beta cells, so the pancreas can no longer produce insulin at all. People with Type 1 must inject or pump insulin externally just to survive. In Type 2 diabetes, the pancreas still produces insulin but the body's cells become resistant to it — and over time, beta cell function can decline further.
Diabetes affects over 500 million people worldwide, which is a stark reminder of how critical this one small organ really is.
A Discovery That Changed Medicine
Before 1921, a Type 1 diabetes diagnosis was essentially a death sentence. That year, Canadian scientists Frederick Banting and Charles Best successfully isolated insulin from a dog's pancreas. Within a year, it was being used to treat humans. It remains one of the most important medical breakthroughs in history — all tracing back to understanding what the pancreas does.
